In order to create a healthy living and learning environment for students, faculty, and staff, Aquinas College requires every student to provide proof of immunizations before moving into campus housing or the commencement of class.  Please see the information below from the CDC that supports our decision to make this requirement: https://www.cdc.gov/vaccines/vac-gen/whatifstop.htm.


Each new student must return the Aquinas Health & Immunization Record.  Click here for the form.

Before sending your form, please make sure that the form has been signed by the student, parents/guardian and your healthcare provider.

The completed Aquinas Health & Immunization Forms must be sent to:
